Founded in 2012, Urban Street Angels is a San Diego based non-profit dedicated to supporting homeless youth through supportive housing, peer support, mentorship, mental health counseling, and job training initiatives, to develop a more employable workforce and cultivate personal responsibility. Of California's unhoused population, 50% cite they first had an experience with homelessness between the ages of 18 and 25. This critical demographic has the potential to leave the streets behind for good when provided with the support needed to make a change in their lives. Title : Health & Wellness Coordinator

Position Summary : The Health & Wellness Coordinator works with transitional-age youth (18 - 25) in the organizations Just Be You (JBU) Program, providing housing and recuperative services in a holistic environment, in partnership with the County of San Diego. The incumbent will balance two main service deliveries, health and, wellness. The position will manage a range of activities, requiring close monitoring, follow-up, and advocacy for each youth. The Health & Wellness Coordinator will function as a Peer Support Specialist when holistic services have been completed.
Responsibilities :
Coordinate scheduling for the youth in the day-to-day operation of JBUs holistic and recuperative servicesAbility to multi-task and handle competing demands from program participants with the ability to use training and remain calm during any potential emergencies or crisesProvides education and support to clients about the mental health stigma and ways to combat it in regards to the working environmentHandles move ins & move outs from program including setting up and flipping a room / bedTransports clients as neededPlans outings and activities for clientsInitiate and coordinate the care plan, providing educational information in conjunction with direct care providers related to treatments, procedures, and continuing care requirementsReview the entire care plan for each youth in conjunction with the direct care providers, to include monitoring of deviations from the expected plan of care, and interacting with involved departments to negotiate and expedite scheduling and completion of program milestones, especially health and wellness related milestonesCollaborate effectively with the Program Manager, Occupational Therapist and others in the case management team to develop a multi-disciplinary holistic care planMaintain case documentationParticipate in the case management process by participating in meetings, coordinating information and care requirements with holistic care providers, working to resolve issues that could affect care progression, providing education to others regarding the process, and fostering peer supportRespect program participants by recognizing their rights, and maintaining their confidentialityServe as liaison between partners and linked programs and encourages clients use of these resourcesEstablish and maintain collaborative partnerships with local homeless providers, local Continuum of Care, and other stakeholders in the communityAdvocates for program participants by educating about grievance or complaint proceduresQualifications :
Undergraduate degree or enrollment in undergraduate program preferred (Required for some grants; medical assistant or nursing experience a plus)Experience in healthcare settings and / or social services sectorComfort and competence in digital record keeping and basic data inputExperience managing sensitive personal health information, including basic HIPAA complianceExperience engaging regularly and collaboratively with social service personnelExperience working with substance use disorders (SUD) and severe mental illness (SMI)Six (6) months working with underserved populations preferred (may be volunteer based and cumulative)Must have active CA drivers license and clean driving recordAbility to advocate effectively and maintain professional boundariesAdditional Requirements :
