    • Your Journey as a Case Manager Awaits

  • Collaborate with an interdisciplinary team to tailor treatment plans that align with patient needs.
  • Establish both tentative discharge and contingency plans to enhance patient care.
  • Oversee discharge experiences, facilitating team meetings and treatment adaptations.
  • Document case management addendums and maintain compliance with regulations and policies.
  • Engage with patients and families upon admission to foster a supportive communication plan.
  • Ensure efficient resource planning to avoid discharge delays.
  • Monitor compliance and make timely referrals to post-discharge providers.
  • Participate in utilization reviews, collecting data, and analyzing trends for continuous improvement.
  • Be a part of the on-call case management team when needed.

    • Qualifications to Join Our Team

  • Licensure or Certification :
  • Must complete assessments within their professional scope (e.g., RN, SW, PT, etc.) and hold a current active license if required.
  • Eligibility for CCM® or ACM certification must be met upon entry or within two years.

    • Minimum Requirements :

  • If you're a Nurse, you need at least an Associate Degree in Nursing, with a BSN preferred. A diploma is acceptable in states with diploma requirements.
  • Other healthcare professionals must possess a minimum bachelor’s degree, with a graduate degree preferred.
  • At least two years of rehabilitation experience is preferred.
