Our Client is seeking for a Nurse Case Manager is responsible for overseeing the patient’s plan of care from admission through discharge. By collaborating with physicians, nursing staff, and ancillary departments, this individual ensures timely access to services, removes barriers to care progression, and advocates for the patient’s best outcomes.
Key Responsibilities
- Coordinate individualized care plans in partnership with patients, families, and clinical teams.
- Facilitate smooth transitions of care by anticipating and addressing service needs throughout the hospital stay.
- Monitor patient progress, evaluate the effectiveness of interventions, and identify potential delays or variances in treatment.
- Collaborate with physicians, nursing, and ancillary services to ensure continuity and efficiency of care delivery.
- Serve as the patient’s advocate—communicating healthcare needs and ensuring understanding of the care plan.
- Support compliance with hospital quality initiatives and utilization standards.
Qualifications
Education : Bachelor’s degree in Nursing preferred.Experience : Minimum of 3–5 years of nursing experience in an acute care setting (Staff Nurse II level or higher).Prior experience in case management, utilization review, or quality improvement strongly desired.Licensure : Active Registered Nurse (RN) license in the State of Texas required.Certifications : Case Management certification (ACM, CCM, or ANCC) preferred; must be obtained within two years of hire.Current Basic Life Support (BLS) certification from the American Heart Association preferred.
