Description :

Work with an interdisciplinary mental health treatment team.

Complete screenings which include detailed mental health screens.

Conduct individual and group clinical interventions as prescribed on the individual mental health treatment plan.

Utilize clinical intervention strategies within their scope of practice and directed by supervision.

Provide psychosocial group interventions as prescribed on the mental health treatment plan.

Deliver mental health liaison services to an assigned caseload.

Monitor the progress of individuals with the mental health treatment plan goals.

Complete psychosocial needs and treatment assessments as part of interdisciplinary treatment planning.

Provide and seek information related to patient care and treatment.

Make appropriate community referrals in accordance with the mental health treatment team.

Write reports and / or documentation, including electronic documentation of all inmate contacts.

Document mental health liaison progress note entries, discharge plans, and miscellaneous forms and reports as needed.

Attend meetings for social work or counselor supervision.
