SUMMARY

The Community Family Intervention provides social / emotional behavioral health services to children and families. These services may be provided in school, home, and community settings.

Services include but are not limited to : individual and family therapeutic supportive and social / emotional skill building services, crisis intervention, prevention, consultation and education, and linkage to needed community resources.

This position reports to the Community Family Intervention Manager.

PRIMARY DUTIES AND RESPONSIBILITIES :

  • Contributes to creating a welcoming and inclusive environment for our clients, colleagues and community partners from diverse backgrounds and experiences.
  • Integrates principles of Trauma Informed Care in all aspects of work.
  • Is sensitive to the children’s culture and socioeconomic characteristics that delivers the best possible care to the youth and families served.
  • Provide activities and intervention to children and families participating in the program to build social and emotional skills.
  • Develop Individualized Treatment Plans with youth in collaboration with the youth and families and identify interventions and resources to help youth meet treatment goals.
  • Complete assessment tools to be used to measure progress in treatment.
  • Providing behavioral health services to a caseload of 15 to 18 youth and their families.
  • Provide prevention, consultation, and education services to school staff, youth, and families.
  • May assist in de-escalation strategies to support youth and families in crisis.
  • Provide billed services at a minimum of 90% of the stated Agency expectation on a monthly basis.
  • Maintain accurate documentation of direct and indirect services provided in accordance with ODMH, ADAMH, and agency standards.
  • Participate in staff and team meetings for the schools and agency, staff development and in-service training, planning interventions and regular supervisory conferences.
  • Other duties as assigned or requested.

EDUCATION and / or EXPERIENCE

Bachelor’s Degree in a Human Service-related field such as psychology, sociology, human services, diversity studies, human development, social work, education, early childhood, other related discipline required.

Minimum of two years of experience working with children and families preferred, but not required. Valid Driver’s License and Proof of Auto Liability Insurance.

Rendering Provider number required based on level of licensure. Licensure is not required. National Provider Identification number required (the agency will provide instructions for obtaining if needed.)

  • Ability to maintain high degree of empathy and compassion in meeting the needs of agency clients and client families.
  • Ability to foster open, honest and candid communication.
  • Ability to multi-task and maintain organization in a fast paced, changing environment.
  • Ability to maintain flexibility in managing change.
  • Ability to successfully operate where ethical decision will be required and utilize supervision appropriately.
  • Ability to create and maintain highest levels of confidentiality when dealing with client information, SVFC proprietary information and sensitive situations.
  • Ability to complete and turn in paperwork in a timely fashion.

LANGUAGE SKILLS

Ability to effectively communicate plans, goals, directives and diagnosis information between provider and clients.

MATHEMATICAL SKILLS

Ability to calculate figures and amounts using basic math skills or use of a calculator.

  • Ability to work some evening hours due to the service delivery and administrative needs of SVFS clients and families.
