Requirements :

• Graduate of an accredited Physical Therapy program
• Active PT license in CA
• Minimum 1 year of recent Home Health experience
• EPIC EMR experience is a must
• BLS certification (AHA)
• Valid Class C Driver’s License & Auto Insurance required for field visits
