Key Responsibilities :

• Assist residents with basic healthcare needs, including diet reminders, medication management, and exercise encouragement.
• Observe and document resident behavior, mood, and home adjustments for caseworker reporting.
• Offer emotional support and companionship during times of loneliness or grief.
• Maintain clear records of your observations and interactions with residents.
• Communicate regularly with nursing staff or home health directors regarding resident needs.
• Promptly report any emergencies to nursing or home health offices.
• Confirm scheduling of visits weekly with coordinating personnel to ensure seamless care delivery.
