Job Description

American Traveler is seeking an experienced RN for a Home Health position requiring OASIS documentation proficiency and a VT or compact RN license.

Responsibilities

  • Work in a home health setting providing direct care to patients in their homes
  • Day shifts from 8am to 4pm with options for Monday to Friday, Tuesday to Saturday, or Sunday to Thursday schedules
  • Some weekend and on-call responsibilities are required
  • Patient caseload includes a variety of complex needs and high-tech cases such as IV therapy and complex wound care
  • Utilizes OASIS documentation and follows agency policies for visit documentation
  • Acts as case manager managing caseload and visit times, ensuring compliance with agency productivity standards
  • Completes patient and caregiver education on medications, treatments, and discharge instructions
  • Coordinates care with physicians, providers, and the broader care team to ensure effective patient care
  • Responsible for timely and accurate completion of all required documentation and forms
  • Participates in team meetings, peer reviews, and interdisciplinary conferences to develop and monitor individualized care plans
  • Ensures infection control and quality of care throughout the plan of care
  • Requirements

  • Active VT or compact RN license required
  • Minimum 2 years of home health nursing experience
  • Proven proficiency with OASIS assessment and documentation
  • Strong skills in case management, high-tech nursing interventions, and collaborating with interdisciplinary teams
