Position Summary :

Sidney Health Center is seeking a Care Management RN or LPN to join our I CARE team that provides a flexible schedule. The Care Management Nurse will work in collaboration with primary care clinicians and healthcare teams. They act as service liaison, ensuring patients receive comprehensive and coordinated care and maintain accurate and up-to-date timely records of patient interaction, care plans and changes in health status in the EHF, coordinate and manage high-risk, chronically ill patients through Chronic Care Management, utilizing health coaching techniques to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk. Essential duties of the position include excellent people skills, organizational skills, and the ability to work well with all staff.
