Physical Therapy Assistant

Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help "homebound" patients recover from illness or injury and manage exacerbations of chronic disease. We are seeking a full-time, 36-hour Physical Therapy Assistant to join the "Minuteman" team. While the majority of your time will be spent working in Framingham and Waltham, the entire team also supports the following territories : Ashland, Fayville, Hopkinton, Hudson, Marlborough, Southboro, Sudbury, Wayland, Weston. The desired schedule is Monday through Friday. Anticipated daily volume is 6-7 visits per day plus chart review, visit preparation, documentation, and care coordination.

Provide physical therapy services, under the direction of a registered physical therapist to patients in their place of residence, who have physical impairments, resulting from injury, disease, disability or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.
