About The Position

Job Title : Inpatient Case Manager

Profession : RN

Experience : Minimum of 2 years acute clinical nursing experience preferred.

License : Current state RN license.

Certifications : Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.

Must-Have :

  • Graduate of an accredited nursing school or college.
  • Minimum of 2 years acute clinical nursing experience preferred.
  • Case Management experience strongly preferred.
  • BSN / MSN preferred.

Description :

The Inpatient Case Manager assesses, plans, implements, monitors, and evaluates options and services to create an appropriate, individualized plan for hospital patient care across the continuum of care.

The Inpatient Case Manager will follow assigned patients while working with team members and other services to prioritize and strategize daily activities.

Using independent judgment and discretion, patients must be assessed and reassessed as part of an ongoing process to address their broad spectrum of needs.

The Inpatient Case Manager utilizes clinical knowledge and competence, communication skills, problem-solving, and conflict resolution techniques.

Strong skills in assessment, organization, and time management are essential.

The Case Management process requires a focus on critical thinking, customer service, setting appropriate goals, and measuring outcomes to ensure optimal patient outcomes with consideration to financial and health plan resources.

The role involves preparing and issuing disbursements with proper authorization in a timely manner.

Ensuring the accuracy of amounts to be paid and proper coding is critical.

Responsibility includes maintaining all vendor accounts, reviewing, matching, coding, and key punching invoices.

Researching problems and working with vendors and staff in a professional, timely manner is essential.

Maintaining vendor accounts by reconciling statements monthly is required.

Accurately accruing sales tax for out-of-state vendors is necessary.

Producing weekly check runs and associated reports is part of the job.

Matching checks to invoices and verifying invoice detail to check detail is also required.

Assisting management with other duties as assigned may be necessary to meet business needs.

Physical Effort :

The physical demands include long hours of sitting and lifting up to 35 to 40 pounds.

The role requires some walking, stooping, bending, twisting, periods of sitting and standing, and use of a computer keyboard throughout the workday.

The ability to be organized and efficient in prioritizing and managing assignments with minimal oversight is important.
