A company is looking for a Telephonic LPN Care Coordination.

Key Responsibilities

Conduct telephonic outreach to coordinate care, admissions, and services

Support transitions of care for members entering or discharging from long-term care or rehabilitation settings

Document all interactions and ensure compliance with HIPAA and organizational protocols

Qualifications

Active, unrestricted LPN license in the State of New Hampshire

Minimum 2 years of nursing experience in long-term care or care coordination

Familiarity with New Hampshire Medicaid processes preferred

Proficiency in EMR systems and care coordination technology

Ability to work independently in a remote setting
