Location : Fully Remote - Must reside in HI, TX, IL, FL, or PA and hold an active pharmacist license in that state
Duration : 06 months contract
Key Responsibilities
Conduct telephonic comprehensive medication reviews (CMRs), ensuring full medication reconciliation and completing condition-specific clinical assessments.
Identify, document, and coordinate resolution of medication-related problems.
Prepare patient-friendly written summaries (takeaways) for each completed MTM session.
Create and send faxed clinical documents to prescriber offices to support intervention recommendations.
Build and maintain strong relationships with physicians, nurses, internal teams, and external partners.
Perform additional duties as assigned by leadership.
Required Qualifications
Bachelor's degree in pharmacy or PharmD from an accredited institution.
Active, unrestricted pharmacist license in HI, TX, IL, FL, or PA.
Must reside in the licensed state.
High School Diploma or GED (verifiable).
Ability to work remotely with :
A dedicated, quiet workspace compliant with HIPAA
Hard-wired internet connection (ethernet required)
Screenshot of internet upload / download speeds
Screenshot of active pharmacist license
Completion of questionnaire including brief description of past call center experience.
Preferred Qualifications
Previous MTM experience, ideally using Outcomes MTM / MTM Connect.
Strong organizational, communication, and documentation skills.
Experience working in a remote environment with productivity targets.
Proficiency with Microsoft Office and basic computer navigation.
Previous call center experience (inbound or outbound).
