FULL-TIME HOME HEALTH RN CASE MANAGER (RN or LVN) – NOW HIRING!

We are seeking a compassionate and dedicated Full-Time Home Health RN Case Manager (RNCM) to join our team! If you are passionate about providing quality care and have strong clinical skills, we want to hear from you.

✨ Responsibilities

• Provide skilled nursing care under the supervision of the Registered Nurse/ADON
• Deliver compassionate home health care to geriatric patients in their place of residence
• Coordinate care with the interdisciplinary team, patient/family, and referring agency
• Ensure accurate and timely documentation
• Maintain compliance with all home health standards and regulations

✅ Requirements

• HOME HEALTH experience required
• RN or LVN license in the State of Texas (active and in good standing)
• MUST HAVE KINNSER experience
• Valid driver’s license and auto insurance
• CPR certification
• Strong communication and organizational skills
• Commitment to compassionate, patient-centered care
