Reason for coverage : Loss of provider, we have patient panelsSchedule : Our Clinic is open for Provider schedules 830 7 M,T, H, F, on Wednesdays, and 830 2 on SaturdayShift : We are flexible within the above time frames.Call : NoPatient volume : For a full day, around 17Scope : 100% Family Medicine EMR : Next GenHospital : Referrals to Salem Health, Silverton (Legacy) or Stayton (Santiam) hospitals depending on where a patient lives.Group / support : We have a cadre of nurses, medical assistants, and other licensed medical providers on siteRequirements (boards, license) : Licensed to practice in Oregon as MD or DO with DEA designation .
