Job description

Trinity Staffing Group is seeking a travel nurse RN Skilled Nursing Facility (SNF) Skilled Nursing for a travel nursing job in Anchorage, Alaska.

Job Description & Requirements

  • Specialty : Skilled Nursing

Post Acute Facility

  • Perform initial assessments on admission — evaluate physical, mental, skin integrity, mobility, nutrition status, etc.
  • Monitor vital signs, fluid balance, weight, input / output.
  • Observe for changes in condition (e.g. signs of infection, pain, adverse reactions) and report to physician or advanced practice provider.
  • Monitor wounds, risk of falls, skin breakdown.
  • Work with interdisciplinary team (physicians, therapists, dietitians, social services) to create individualized care plans.
  • Update care plans as patient's needs change.
  • Ensure therapies (PT / OT etc.) are integrated into plan.
  • Discharge planning : coordinate with patient / family to transition to home or next level of care.
