Geriatrics Nurse Practitioner - Now Hiring

Seeking a Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients just north of Providence, Rhode Island.\n\nOpportunity Details : \n

• Full-time, Mon - Fri, 8am-5pm\n
• 13-16 patients per day\n
• Geriatric population\n
• Provision of exceptional primary care.\n
• Conduct office visits for routine and acute issues.\n
• Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.\n
• Care coordination with other providers, specialists, testing facilities, and agencies.\n
• Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)\n
• Assisting the care team with phone triage and outreach.\n
• Conducting home visits as needed\n\nQualifications : \n
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required\n
• National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC\n
• Experience in primary care : Internal medicine, geriatrics, or family medicine preferred\n
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).\n
• Bilingual proficiency in Arabic required\n\nCompensation & Benefits : \n
• Competitive Salary - $105,000-$148,500 Wider range depending on experience\n
