Physician - Neurology Tacoma

Summary : General Neurologist- backfill for a credentialing drop out

Start : ASAP, most likely late Jan due to credentialing

First Day : Unit orientation will be held prior to patient care. First day of seeing patients will be very light, with EPIC support on site.

Rate Caps for Submission :

Malpractice - Must be included in rates.

Orientation - half the hourly / callback rate

• kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"

Hourly -

Holiday - Must be 1.5x the regular rate

Submissions over the cap will not be cleared by MSP.

Schedule / Availability Requirements :

Monday-Friday, 40 hours per week consisting of 36 hrs patient contact and 4 hrs administrative. 7a-5p. September through Jan / Feb with possibility of extending depending on recruitment success in the region.

Facility Location : Franciscan Neurology Associates- Tacoma

Setting : Clinic

Duties : New patients, follow up visits, therapeutic treatments (Botox, ONB. etc.)

FTE : 1.0

Days per month : 5 days / week

Required Procedures : Minimal, Botox, ONB, etc.

Patient Volume : 13-15 per day

EMR : EPIC

Support Staff : Established LPN or MA already in clinic

Reason for Coverage : Hospitalist model changing, excluding ambulatory visits

Deal breakers : FYI : work area is pod style with all staff in close proximity. Administrative work stations are available.

Timeframe : 45-90 days

Requirements : MD / DO General Neurologist

Credentialing with other facilities? automatically credentialed at all but St. Michaels

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?

SUBMISSION REQUIREMENTS

Board Certification - Board Certified, Board Eligible - Highly Preferred

State License - WA or IMLC - Highly Preferred

Other Certifications - BLS, DEA - REQUIRED

Malpractice - Clean Preferred

Availability at time of name clear - REQUIRED 60 days of availability from start date. Specific dates and black out dates required for presentation

Required Documents for a Full Presentation Packet :

1. Up-to-date CV (Gaps greater than 30 days will need notation / explanation of gap in work history)

2. Malpractice documentation (NPDB and or write up). Must include ALL cases, dates, and payouts.

3. Disciplinary Actions (Must provide a detailed explanation)

4. Copy of Board Certification(s)

5. LOQ / State License

6. Supporting certifications such as DEA, BLS, ACLS, ATLS, NRP, PALS

7. Copy of clean Sex Offender search
