Are you passionate about helping individuals overcome addiction? MedMark Treatment Centers, a BayMark Health Services company , is seeking a dedicated Substance Abuse Counselor to provide high-quality care to patients undergoing medically assisted treatment for opioid addiction . As a counselor, you will play a vital role in guiding patients through recovery , offering coping strategies , and collaborating with healthcare professionals to ensure successful outcomes. Key Responsibilities :

• Conduct individual assessments and evaluations to determine service eligibility.
• Complete Bio-psychosocial assessments to create personalized treatment plans.
• Facilitate intake processes and ensure smooth patient onboarding.
• Develop and review individual treatment plans tailored to each patient s needs.
• Provide individual and group counseling services at regularly scheduled intervals.
• Create aftercare and discharge plans to support long-term recovery.
• Collaborate with physicians, nurses, and fellow counselors to optimize patient care.
• Maintain accurate patient records and documentation in compliance with state and federal guidelines.

Qualifications :

Salary & Benefits :

Hourly Pay : $27.00 - $28.25 (based on experience, education, and specialty)
