Primary Care Physician – PACE Program

Our client is seeking a dedicated Primary Care Physician to provide comprehensive, compassionate care for participants enrolled in their PACE (Program of All-Inclusive Care for the Elderly) program. This position plays a key role in promoting participants' health and independence by coordinating medical care and collaborating with an interdisciplinary team (IDT) to ensure the highest quality of care.

Position Highlights :

• Deliver comprehensive, coordinated medical care guided by the PACE benefit model.
• Conduct initial, semiannual, annual, and periodic participant assessments.
• Partner with the interdisciplinary team to develop, review, and implement individualized care plans.
• Oversee diagnostic testing, medication management, and appropriate specialist referrals.
• Provide follow-up care and collaborate with participants' families and support networks.
• Participate in team meetings, care conferences, and family discussions as needed.
• Coordinate care for hospitalized and nursing home participants, ensuring smooth transitions and adherence to care plans.
• Participate in quality assurance and performance improvement programs (QAPI).
• Document all care in eClinicalWorks (EMR) and maintain compliance with all policies, procedures, and HIPAA regulations.
