Preferred schedule for locum :

Friday : OR : 07 : 00- 15 : 00 1st Call : 15 : 00 - 07 : 00

Saturday : OR : 07 : 00 - 11 : 00 (OR may be extended depending on volumes) 1st Call : 11 : 00 - 07 : 00

Sunday : OR : 07 : 00 - 11 : 00 (OR may be extended depending on volumes) 1st Call : 11 : 00 - 07 : 00

In addition to weekend coverage, the following dates are also needed :

• October 30 Nov 3; 7a 7a
• Nov 25 Dec 1; 7a 7a
• Dec 16 Jan 5; 7a 7a

Credentialing Information :

Timeframe : 90 days - need to fully Health Good Samaritan & with CHI Health Partners
