Peer Specialist

JOB DEFINITION :

The Peer Specialist functions as a member function as a member of the Street Outreach Team to serve individuals experiencing homelessness located in and around Long Island Railroad (LIRR) stations across Nassau County. Advocates for and supports the client, engages with community agencies / health care. Peer Specialist will serve as a role model and / or health coach for clients.

EDUCATIONAL REQUIREMENTS :

High School Diplomas or GED.

EXPERIENCE REQUIRED :

• Relevant life experience required : personal experience living with a disability and / or social / emotional challenge, and willingness to draw upon this experience to serve as a role model to support the recovery model within Health Home model.
• Working knowledge of computer software and electronic health record systems.
• Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
• Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.
• Familiarity with NYS Entitlements, Housing, Health, and / or Mental Health / Substance Abuse Service Delivery System.
• Knowledge of Medicaid, Social Security and other entitlements preferred.
• Excellent interpersonal skills required.
• Ability and willingness to regularly travel to many locations, sometimes with clients in the vehicle, using various modes of reliable and safe transportation.
• Ability to maintain a clean and valid New York State driver's license.
• Ability to safely operate a vehicle in a safe manner under all driving conditions.
