Nurse Practitioner, Specialty - Medical Office, Nephrology

Work Site Locations : Upland, West Covina and Apple Valley. May be required to travel to other clinics for business need.

Work Shift : 5 Days a Week, Monday – Friday : 8 : 00 am - 5 : 00 pm

The Nurse Practitioner Specialty is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and / or evaluation. Performs care in the specialty area of nephrology and patients on dialysis.

• Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions.
• Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations.
• Acquires patient health histories and evaluates patient, performing physical examinations.
• Orders, interprets and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.
• Discusses case with physician.
• Formulates and documents care plan.
• Prescribes medication, therapies and / or other forms of treatment.
• Evaluates the patient’s response to therapy.

Compensation : $132,756K - $165,945K & bonus eligible
