Job Responsibilities :

• Manage a panel of established patients ensuring high-quality primary care services.
• Conduct chronic care management for conditions such as diabetes hypertension and thyroid disorders.
• Collaborate with medical assistants to support patient care and clinic operations.
• Maintain accurate and timely documentation in the Cerner CareB EMR system.
• Attend to adult patients (ages 18 and up); pediatric care is optional.
• Fulfill a full-time schedule from Monday to Thursday from 7 : 30 am to 5 : 30 pm.
