Qualifications :
Valid and current LPN license in Virginia.Compassionate and patient-centered approach to care.Strong communication skills and ability to work collaboratively within a team.Attention to detail and commitment to delivering high-quality care.Summary : Under direct supervision of the Director of Nursing (DON), performs clinical duties related to nursing care. Promotes the welfare and dignity of all residents and acts as an advocate in the delivery of care; promotes the mission and values of Martha Jefferson House both internally and within the community; complies with the Code of Conduct and supports the Corporate Compliance Program.
Essential Functions :
Administers care, treatment, and medication to residentsObtains report from the previous shift and reports to the on-coming shift.Makes out daily staff assignments, including resident care, specific instructions, breaks, mealtime, and environmental assignments (clean tub room, clean dining room, etc.)Frequently makes rounds to ensure resident care, treatment, and conditionCall physician with follow-up updates and needsUpdate MARs, ADLs, and treatments daily or as neededRecords and implements physician orders and monitors resident's medical conditionAssign CNA to weigh resident on a daily / weekly / monthly basis as ordered and review weights with follow-up as neededResponsible for communicating changes of condition of the resident to family member / POA, DON, Administrator (as required) and physician in a timely mannerResponsible for overseeing the completion of incident reports, notification of family / POA, MD, DO, and Administrator (as needed)Ensures that documentation, monthly and quarterly assessments, progress reports, summaries, and assessments are complete and done correctlyAssists in the training and supervision of new employeesOperates medical equipment and emergency equipment and checks treatment cart for suppliesHelps with resident feeding as neededCompletes the 24-hour shift reportNotifies the DON on an ongoing basis of changes, family concerns, interdepartmental concerns, staff concerns, any deviation from normal activity of resident function, environment, or staffPerforms supervisory duties as requiredOther duties as assignedCarries out the facility's disaster preparedness plan in cooperation with an on-duty security guardWe are an Equal Opportunity Employer.
