As an LPN for the Home Health team, the partner will perform nursing services that require the use of physical, social sciences and nursing skills in the care of the sick, the conservation of health and the prevention of disease in patients home. Responsible for the care of the patients assigned as directed by the registered nurse. Other duties as assigned.

Essential Duties and Responsibilities

Education and Experience

Graduate of an accredited LPN Program OR

Wisconsin : Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing.

Illinois : Have completed the designated curriculum of a Ladder Option Approved Designated RN Program.

A minimum one year relevant experience preferred.

Certification and Licensure

Current Illinois or Wisconsin licensure by the IL Depart of Professional Regulation (IDPR) and / or Wisconsin Department of Safety and Professional Services required

Approved CPR or BLS certification required within 90 days of hire

Maintain a valid driver's license.

Have and maintain an acceptable driving record per Mercyhealth's HR.100 Fleet and Driver Safety policy.

For those driving their personal vehicles :

Maintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.
