• 40 hours per week full time schedule available
• 30 units per week productivity requirement
• Home health setting providing care to adult patients
• EMR : Homecare Homebase
• Focus on neuromuscular, neurological and orthopedic disorders
• OASIS documentation experience needed
• Home health care experience required
