Collaborate with physicians and the interdisciplinary team to ensure patient-centered care and optimal outcomes.\n\nWhat You Will Do\n
- Provide expert assessments and recommendations to physicians based on comprehensive evaluations of patients' physical and emotional needs.\n
- Utilize the nursing process to conduct Start of Care visits, post-hospital evaluations, patient re-certifications, discharges, and supervisory visits.\n
- Collaborate with physicians and the healthcare team to develop individualized care plans.\n
- Make informed recommendations for resource utilization for effective outcomes, implementing clinical protocols based on patient diagnoses.\n
- Respond to requests for changes in care plans and assess their significance to ensure proper documentation.\n
- Educate and guide patients and caregivers on disease management and effective self-care practices.\n\nEssential Qualifications\n\nTo thrive in this role, you must be able to perform essential functions satisfactorily and possess the ability to communicate effectively, with the necessary visual acuity and auditory skills.\n\nRequired Education, License / Certification, or Work Experience : \n
- Registered Nurse (RN) - Board of Nursing Upon Hire\n\nPreferred Education, License / Certification, or Work Experience : \n
