Opportunity Description

Immediate need for an experienced Social Worker to support multiple facilities in the Syracuse NY area.

Job Duties

Assigned to provide coverage as needed at multiple facilities

Provides comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and their families.

Addresses social determinants of health and works collaboratively with the multidisciplinary team.

Experience & Skills Required

3 years experiencen in a hospital or healthcare setting

Masters Degree
