Job Summary :

Position Title : Social Worker

Location : Glasgow, KY 42141

Duration : Open - ended

Work Schedule : Monday - Friday 8 : 00 AM - 4 : 00 PM

Responsibilities for this position include, but are not limited to :

• Assists with admissions including scheduling and completing appropriate paperwork and distributes to applicable personnel.
• Completes individualized psychosocial assessments and plans of care for new residents and quarterly for all other residents.
• Enters data in computer systems assuring compliance with facility, state, and federal mandated regulation, policies, and procedures.
• Collaborates with the activity department to ensure therapeutic activities are conducted as described in the resident's individual plan of care.
• Participates on committees as assigned. Contacts resident representatives regarding change in condition as needed.
• Education : Bachelor of Social Work or related fields.
