California Licensed Clinical Case Consultant

A company is looking for a Registered Nurse - Clinical Case Consultant (CalAIM ECM Program).

Key Responsibilities

Provide clinical oversight and guidance to care coordinators and case managers, ensuring evidence-based care plans

Support care coordination and seamless transitions from hospital to community settings, identifying at-risk members

Maintain accurate documentation and compliance with Medi-Cal ECM standards while promoting quality improvement initiatives

Required Qualifications

Active and unrestricted Registered Nurse (RN) license in California

2+ years of direct clinical experience in community health, acute care, public health, or behavioral health settings

Experience working with Medi-Cal populations, including those who are homeless or living with serious mental illness or substance use disorders

Familiarity with CalAIM ECM, Whole Person Care, or Health Homes Program is preferred

Bilingual proficiency in languages such as Spanish, Mandarin, Vietnamese, or Tagalog is a plus
