Nurse Care Manager – Managed Long-Term Care (MLTC)

Location : Brooklyn, NY (Hybrid : 4 days remote, 1 day field in Brooklyn after onsite training)

Sign-On Bonus : $20,000 ($5,000 payable every six months, less taxes)

Key Responsibilities :

• Conduct in-home assessments and develop personalized care plans for members.
• Collaborate with members, families, caregivers, primary care practitioners, and interdisciplinary teams to ensure consistent, high-quality care.
• Authorize covered services and coordinate care across different payers.
• Monitor members’ needs and health status to maintain the most independent living situation possible.
• Advocate for members while promoting quality and cost-effective outcomes.
