About the Role :

We’re hiring a full-time Geriatric Physical Therapist to join our interdisciplinary care team in Livermore, CA . You’ll play a vital role in our PACE (Program of All-Inclusive Care for the Elderly) — where your clinical expertise will directly improve mobility, safety, and independence for frail older adults.

What You’ll Do :

• Evaluate the functional ability, mobility, and rehab needs of older adults
• Develop and implement individualized, evidence-based care plans
• Collaborate with physicians, nurses, social workers, and caregivers
• Educate patients, caregivers, and staff on safe mobility and equipment use
• Supervise PTAs and therapy aides within the care team
• Evaluates participants’ home environments and makes appropriate recommendations as needed.
