We are seeking a  Case Manager  to maintain a central role in active engagement, coordination of services and ensures that the patients receive optimal care including chronic disease management, mental health management, and preventative care across multiple health settings and multiple providers.

Responsibilities :

• Monitors cases by verifying clients’ condition; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
• Maintains clients’ records by reviewing case notes; logging events and progress.
• Manage social determinants of health to help reduce incidence of rehospitalization.
• Be assigned to advise seniors who need community resources and have directory of available resources who can assist the seniors.
