A company is looking for a licensed clinical social worker.
Key Responsibilities
Utilize a collaborative process for assessment, planning, facilitation, and advocacy to meet individual health needs
Facilitate healthcare outcomes by assisting with appointment scheduling and accessing benefits
Employ critical thinking and judgment to inform the case management process
Required Qualifications
Active IL LCSW or LCPC license
Minimum 3-5 years of clinical practical experience
Preferred 2-3 years of care management, discharge planning, or home health care coordination experience
Ability to work independently and effectively in a remote environment
Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
