A company is looking for a Registered Nurse II - Case Manager.

Key Responsibilities

Manage continuity of care for beneficiaries through comprehensive assessments and care planning

Coordinate follow-up consult and care services for Service Members

Develop and implement tools and strategies to support case management and ensure quality outcomes

Required Qualifications

Baccalaureate Degree in Nursing from an accredited institution

One of several certifications related to case management or nursing specialty

One year of nursing experience post-graduation

Current, active, and unrestricted Registered Nurse license
