Family Practice - Geriatrics Physician

Job Title : Medical Director - Geriatric Medicine Facility Overview : Join a community-focused PACE program in beautiful Pensacola, Florida, dedicated to serving individuals aged 55 and older. This program supports seniors who wish to maintain their independence while receiving comprehensive medical and supportive services. The Medical Director will work within a collaborative environment promoting dignity and respect for seniors and their families. Job Responsibilities : Conduct comprehensive evaluations of new participants and ongoing assessments. Develop and implement medical treatment plans based on participant needs. Participate in the Intake and Assessment decision alongside applicants and caregivers. Discuss medical conditions and future care plans with participants and their families. Collaborate with an interdisciplinary team to provide holistic health care solutions. Supervise the health care programs designed by the team. Educate participants on health maintenance and chronic conditions. Perform physical examinations, order diagnostic tests, and document findings. Participate in on-call rotation as necessary. Qualifications : Current and active plenary physician license to practice in Florida. Board Certification in Internal Medicine or Family Practice. Board Certification in Geriatrics (highly preferred). Experience working with a frail or elderly population (highly preferred). CDS Certification and DEA Registration. Previous supervisory experience. Minimum one year of experience in geriatric medicine. Strong physical assessment and chronic disease management skills for older adults. Commitment to working in a creative health care setting. License and Certification Requirements : Active Florida State Medical License (required). Board Certified in Internal Medicine or Family Practice (required). CDS Certification and DEA Registration (required). Board Certification in Geriatrics (preferred).
