Internal Medicine Physician
Location : Woodbury, New Jersey
Position Type : 1 Year Contract (Onsite)
Work Arrangement : Onsite - Weekly visits for Medicare Part A residents; Monthly visits for non-Medicare residents
Coverage Requirement : Must ensure 24 / 7 on-call coverage through self or designated qualified individual
The Internal Medicine Physician will serve as the primary medical provider for nursing home residents. This role involves :
- Providing direct care and clinical oversight.
- Reviewing and managing residents' total care plans and medications.
- Ensuring regulatory compliance with local, state, and federal standards.
- Documenting all clinical activities and treatment plans within the Sigma Care System (EHR) .
- Participating in Interdisciplinary Care Team (IDCT) meetings and care coordination efforts.
- Maintaining valid credentials, licensure, and malpractice insurance.
Key Responsibilities
Provide medical care and treatment to all residents (Medicare and non-Medicare).Conduct weekly rounds for Medicare Part A residents and monthly visits for non-Medicare residents.Review and document residents' plans of care, medications, and treatments at each visit.Complete and sign all admission and re-admission documentation within 48 hours .Perform monthly medication regimen reviews , aiming for ≤9 medications per resident.Document all notes, orders, and updates in the Sigma Care System .Ensure 24 / 7 on-call availability (directly or via designated provider).Participate in IDCT meetings and coordinate care with nursing and administrative staff.Ensure full compliance with federal, state, and facility regulations .Maintain and submit copies of medical license, CDS certification, and malpractice insurance to the facility.Bill directly to appropriate payers : Medicare, Medicaid, managed care, or private insurance .Education, Experience & Qualifications
Required :
Active New Jersey Physician LicenseValid CDS License (Controlled Dangerous Substances)Active Malpractice Insurance coverageExperience in long-term care, geriatric, or nursing home settingsFamiliarity with Medicare / Medicaid documentation and billing regulationsProficiency with Sigma Care System or other Electronic Health Record (EHR) systemsPreferred :
