The Opportunity

We are seeking a full-time

Home Visit Nurse Practitioner

to join our One Medical at Home, high-risk care team and our High Risk Care Management virtual team, supporting our patients in the

Metro West Phoenix area in Arizona

• . The Home Visit Nurse Practitioner will care for geriatric, frail, and clinically complex patients through a team partnership with the High Risk Care Medical Director and high risk teams, as well as the in office Primary Care Providers.

What you'll likely work on

Rediscover the "joy in practice" while working in a value-based care environment with a team to support you in delivering high quality patient care

Once through orientation, independently manage a panel of approximately 150 case mix adjusted patients with multiple, complex conditions through a balance of home visits and telephone / video interventions

Complete thorough initial visits with patients, diving deep into medical, social and behavioral context

Work directly with the patient and through the multidisciplinary care team to impact ongoing care with particular emphasis on preventable hospitalizations and ensuring care aligned with patient goals

After completing orientation, you will start by conducting in-home or facility visits (such as assisted living, independent living, or group homes) for 4 days each week. After at least 90 days of independent home visits, you may become eligible to conduct telehealth visits one day a week for either scheduled or urgent patient appointments

Collaborate and coordinate with the patient's primary care team and specialists. Participate actively in team huddles to review care plans and assess patient goals of care and care plans

With support of the team, respond to patient and patient's family telephone calls

Participate in the market after-hours call pool

What you’ll need

Completed an accredited NP or PA program with a national certification (ANCC, AANP, NCCPA) in Family, Geriatrics, or Adult

5+ years of experience as a Nurse Practitioner or Physician Assistant with at least 2 years of home based, in office or virtual complex care, Geriatric Primary Care, geriatrics or palliative care experience

Bilingual in Spanish and English highly preferred

Currently licensed in the state of Arizona

Experience with home-based care preferred, specifically in the Phoenix metropolitan area

Experience with value-based care principles and risk adjustment

The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups

Strong technology skills and ability to use telehealth platforms

This is a full-time virtual care role supporting Metro West in Phoenix, Arizona.
