Job description
A company is looking for a Pediatric Care Coordinator.
Key Responsibilities
Receive incoming calls from members regarding benefits, care coordination, and program inquiries
Manage an assigned member caseload requiring short-term care coordination and health navigation
Perform outreach calls to confirm services are in place and document calls accurately
Required Qualifications
Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training / experience
Minimum of 1 year working with medically fragile children in the community
High School diploma or GED
Experience in a fast-paced environment handling multiple priorities
Previous relevant medical training or work experience
