Responsibilities Home hospice and palliative care services in a community-based setting Patient caseload involves assessments, case management, and care for individuals receiving end-of-life support Use of EPIC EMR, remote charting experience strongly preferred Work schedule is day shifts with on-call coverage including overnight and weekend shifts 1-2 times per month Floating to a nearby sister hospital may be required Responsibilities include comprehensive patient assessments, development and evaluation of individualized care plans, and interdisciplinary team participation Role includes medication reconciliation, infection control adherence, and safe medication management Participation required in bi-weekly IDT meetings, monthly staff meetings, and performance improvement activities May provide mentorship or orientation for new employees and cross-train for additional home care skills Must adhere to infection control policies and regulatory standards Holiday and weekend coverage as scheduled Maximum assignment length is one calendar year, with a mandatory 30-60 day break before returning Requirements Active NJ or compact RN license required Minimum of three years of RN experience in a hospice facility setting (not long-term care) Documented hospice travel experience required Current BLS certification required Valid New Jersey driver’s license required Experience using an EMR (preferably EPIC) Excellent critical thinking and communication skills needed 50-mile radius rule : no local candidates considered Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-614780.
