Job Overview - Registered Nurse (Home Health)
Compensation: $95,000 - $107,000/year
Location: Stamford, CT
Schedule: Monday to Friday, 9:00 AM to 5:00 PM (Every Other Weekend)

TAG MedStaffing is hiring a Registered Nurse (Home Health) in Stamford, CT for our client, delivering skilled nursing care to patients in their homes. In this role, you will conduct clinical assessments, administer treatments, and coordinate care plans while ensuring high-quality, patient-centered outcomes. You will collaborate closely with physicians, home health aides, and interdisciplinary care teams while documenting care using OASIS standards.

Responsibilities as the Registered Nurse (Home Health):

• Clinical Assessment & Skilled Care: Perform comprehensive patient assessments and deliver skilled nursing interventions, including wound care, medication administration, and clinical procedures.
• Patient Monitoring & Documentation: Monitor changes in patient condition and accurately document findings, progress, and outcomes in accordance with home health and OASIS requirements.
• Physician Order Management: Execute and follow up on physician orders, communicating changes in patient status and care needs as appropriate.
• Care Coordination & Collaboration: Coordinate care with home health aides, therapists, physicians, and interdisciplinary team members to support continuity of care.
• Quality & Patient-Centered Care: Ensure care delivery meets quality standards while supporting patient safety, comfort, and individualized care plans.

Qualifications for the Registered Nurse (Home Health):

• Licensure: Active Connecticut Registered Nurse (RN) license required.
• Experience: 2+ years of nursing experience required, with prior home health or home care experience strongly preferred.
• Certifications: BLS certification required.
• Technical & Clinical Knowledge: Working knowledge of OASIS documentation and home health care standards.
• Skills & Attributes: Strong communication, organization, and critical thinking skills with the ability to work independently in a home-based care environment.
• Additional Requirements: Reliable transportation required to travel between patient homes within the service area.
