Insurance Verification and Authorization Management :
- Conduct pre-admission commercial insurance verification to determine patient eligibility and coverage.
- Complete post-admission insurance benefit checks to confirm hospice services are covered and documented.
- Submit initial authorization requests for commercial insurance hospice admissions, including follow-up until approved.
- Submit and manage re-authorization requests for ongoing hospice care with commercial payers.
- Track all authorization requests, approvals, denials, and expirations, ensuring timely renewals and updates.
- Maintain and distribute authorization tracking reports to internal departments as needed.
- Initiate and follow through on appeals of authorization denials, coordinating with providers and payers.
Admissions and Medicaid Support :
Assist with the hospice admissions process, ensuring that financial and insurance requirements are completed.Submit and manage Medicaid authorizations, including completion of required forms and coordination with state agencies.Prepare and route 385 forms to designated physicians for completion and submission.Coordinate and submit Medical Record Requests for Long-Term Care (LTC) patients, ensuring timely delivery for claim processing.Patient & Family Support :
Serve as the point of contact for patient and family financial calls, answering questions and addressing concerns about insurance, coverage, and financial responsibilities with clarity and compassion.Educate patients and families about hospice financial policies, including Medicare, Medicaid, commercial insurance, and potential out-of-pocket expenses.Assist families in applying for financial aid programs, Medicaid eligibility, or charity care when applicable.Collaboration and Documentation :
Collaborate with the admissions, clinical, and billing / revenue cycle teams to streamline referrals and resolve financial issues.Maintain accurate documentation of all financial counseling activities, insurance verification, and communication in EMR and billing systems.Stay current with payer requirements, hospice benefit regulations, and changes in healthcare financial policies, and share updates with appropriate staff.Complete daily quality assurance checks following patient admission.Equal Opportunity Employer
