Your Key Responsibilities :

• Attend and contribute to interdisciplinary team meetings, providing assessments as needed.
• Perform initial and ongoing assessments to evaluate participants' functional abilities and rehabilitation needs.
• Recommend adaptive equipment and home modifications to enhance safety and independence for participants.
• Collaborate to develop, implement, and revise individualized treatment plans based on patient progress.
• Provide physical therapy interventions across various settings including clinics, homes, and contracted facilities.
• Educate participants and caregivers on safe equipment use, home safety strategies, and daily living skills.
• Maintain timely and accurate documentation of all assessments and treatment plans.
• Actively participate in daily team meetings, contributing to effective care planning.
• Supervise and mentor Physical Therapy Assistants, ensuring compliance with care plans.
• Implement staff training on physical therapy best practices and equipment usage.
• Support quality improvement and compliance initiatives within our organization.
• Perform other interdisciplinary duties as needed, maintaining professionalism.
• Stay up-to-date with licensure and certifications through ongoing professional development.
• Ensure confidentiality of Personal Health Information (PHI) according to guidelines.
