Our mission is to provide a smooth, supportive, and positive experience for every client — and we’re looking for a Client Care Coordinator who shares that passion for helping others.

About the Role

As a Client Care Coordinator, you’ll serve as the key connection between clients, families, and our clinical and case management teams. You’ll guide clients through the intake, treatment, and discharge process, ensuring their needs are met with empathy, professionalism, and care.

This position is ideal for someone with experience in healthcare administration, behavioral health, or substance abuse treatment coordination, who enjoys creating structure, building relationships, and supporting people during challenging times.

Key Responsibilities

  • Act as the primary point of contact for clients and families throughout their care journey.
  • Coordinate client intake, discharge planning, and follow-up care.
  • Communicate with clinical and case management teams to ensure individualized client support.
  • Maintain accurate, organized documentation in accordance with facility policies.
  • Provide compassionate, professional communication that helps clients feel informed and supported.

    • Pay Range $60,000 to $80,000 annually

    commensurate with experience.

    Requirements : Qualifications

  • Experience in behavioral health, mental health, substance abuse treatment, or healthcare coordination preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Compassionate, client-centered approach with a focus on quality care.
  • Ability to work collaboratively in a fast-paced team environment.
