This RN will coordinate patient care across multiple hospital units to ensure efficient and high-quality transitions of care. Responsibilities Collaborate with interdisciplinary teams to develop and manage patient discharge plans Assess, plan, and coordinate services to meet patient needs across acute and specialty units Communicate with physicians, nurses, and external partners to ensure timely transitions of care Document case management activities using EPIC EHR Float as needed between units at Colby Campus and occasionally to Pacific Campus (1.7 miles away) Assignment & Facility Highlights Facility Type : Level II Trauma Center Beds : 571 (combined campuses) Units : ED, ICU, ITA’s, Peds, Specialty Units, and more Charting : EPIC Dress Code : Business Casual Parking : Facility-paid Requirements License : Active or Pending WA RN License Experience : Minimum 1 year RN experience (Case Management preferred) Certifications : WA RN License required Weekend Rotation : Required (EOW) Call : Not required Float : Required between like units and campuses Locals : Not accepted- Must be 50 Miles from Facility First-Time Travelers : Accepted!
