Primary Function : The Primary function, Monday - Friday (10 a.m. to 2 p.m.), is to provide direct health related care and health monitoring / management, including administering medications, performing treatments, and monitoring vital signs, while also coordinating with families, other healthcare professionals and training direct care staff.

Essential Functions Include :
Participates in Medication Management, including, but not limited to, administration of medication, maintenance of MAR's, timely requests of medication refills, the integrity and orderliness of the medication cart / lockbox, and the appropriate disposition of outdated medications as per policy.Participate in member's intake process ensuring that NJFH receives all necessary medical documentation on each individual prior to the start of programReview medical documentation for each member a minimum of twice per year to ensure it is up-to-date and complete.Maintain documentation as appropriate, including, but not limited to, medical records, appointment schedules and nurses' notes.Provide feedback and information and share concerns regarding supported individuals with the site manager, Day Habilitation Director or other administration.Oversee that the program receives regular medical updates regarding each individual a minimum of 1x per year or more depending on health status.Participate in the formation of a Nursing Care Plan (NCP) for each supported individual if necessary and provide any needed staff training.Provide individual care as directed based on each individual's ISP or NCP.Participate in health training for members, including, but not limited to, recognizing signs and symptoms of medication side effects, disease process and management, infection control & prevention (i.e. handwashing)Monitor vital signs and response to medication and treatments as necessary.Provide guidance, feedback and training to direct care staff based on needs of supported individuals.In collaboration with members' family and staff, monitor chronic diseases and report signs and symptoms of complications.Participate in supportive care, including, but not limited to, chronic pain management, hospice, and palliative care.Ensure that all individuals' medical records are in compliance with all applicable state regulations and requirements of NJFH.Support the interdisciplinary approach to member's care as well as team process and participate as directed by program administration.Provides First Aid / CPR and AED training to agency staff and participates in new employee orientation and training.Provides annual education to members and staff assigned by Program management including but not limited to medication safety, side effects and Winter cold safety, Summer Heat & Sun safety, medications and heat intolerance, etc.Will be required to travel to nearby program sites.Requirements :
Completion of an accredited Practical Nursing / LPN programNJ State LicensureFirst Aid / CPR / AED Instructor certification or ability to become instructor within 60 days of employmentValid driver's license with a clean recordNJFH is an Equal Opportunity Employer.
