GIRMC Discharge Follow-up Advocate

Overview

GENERAL SUMMARY : Follows up with discharged patients and caregivers regarding the plan of care.

Responsibilities

• Commits to the mission, vision, values and goals and consistently demonstrates our core values.
• Understands discharge instructions and ensures access to appropriate care needs.
• Evaluates the understanding of the patient and caregiver about self-care, treatment and rehabilitation.
• Works collaboratively within the multidisciplinary team to promote wellness and optimize patient successful outcomes post-acute hospitalization.
• Assists with arrangement for follow-up health care, assesses financial resources available for patients, and makes referrals to appropriate community resources.
• Assists providers with determining level of care to optimize reimbursement, minimize patient expense, and facilitate quality outcomes.
• Participates in department / institutional performance improvement activities that affect the patient and caretakers to promote service outcomes based on professional practice standards.
• Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
• Participates in meetings, committees and department projects as assigned.
• Performs other related projects and duties as assigned.

Required Knowledge, Skills, and Abilities

Education and Experience

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Minimum of three (3) years of relevant clinical / medical experience required.

Other Credentials / Certifications

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
