Your Role as a Home Health Physical Therapist :

Here’s a snapshot of what you’ll be doing :

  • Deliver home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury.
  • Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care and goals.
  • Observe movements, conduct assessments, document progress, and adjust care plans as necessary.
  • Guide patients through exercises and hands-on therapy to alleviate pain and enhance mobility.
  • Educate patients about their care plans, established goals, exercises, and self-care techniques.
  • Support families by coaching them on the care plan, therapy goals, and safe exercise assistance.

    • What We’re Looking For :

    A few must-haves for our Home Health Physical Therapists :

  • A graduate of an accredited Physical Therapist Program with an active PT license in Michigan.
  • A minimum of 1 year of physical therapy experience, ideally in home healthcare within the last 5 years.
  • Current CPR certification.
  • Familiarity with state and federal home health regulations.
  • Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach.
