This is a Permanant position, Salary range : $48.00 - $51.00, 30 hours M-F with weekends as needed Weekend rotation
The site, established in 2007, is a long-term care facility located on 24 acres in Milford, Delaware and offers skilled nursing care on four units with a total of 144 beds. Our mission is to provide outstanding long-term care services to Delaware veterans that uphold dignity and respect while sustaining and improving their quality of life.
Key Responsibilities
Patient Assessment : Conduct thorough evaluations of patients' physical conditions, including movement, strength, balance, and flexibility. Develop personalized treatment plans based on patient needs.
Treatment Implementation : Provide hands-on treatment to help patients improve mobility, alleviate pain, and regain functionality. This includes exercises, manual therapy, and specialized equipment.
Patient Education : Educate patients and their families about the therapeutic process, exercises to perform at home, and ways to manage and prevent future injury.
Progress Monitoring : Continuously monitor and document patient progress, adjusting treatment plans as necessary to ensure effective outcomes.
Collaboration : Work closely with physicians, nurses, and other healthcare professionals to ensure a coordinated approach to patient care.
Administrative Duties : Maintain accurate and up-to-date patient records in accordance with healthcare regulations. Assist with scheduling, billing, and reporting.
Maintain Professional Knowledge : Stay informed about the latest research and techniques in physical therapy. Participate in training, continuing education, and professional development opportunities.
Qualifications
Licensure : Current state licensure as a Physical Therapist.
Experience : Prior experience in a clinical setting preferred, but new graduates will also be considered.
Skills :
Strong clinical assessment and treatment skills.
Excellent communication and interpersonal skills.
Ability to develop individualized patient care plans.
Knowledge of physical therapy techniques, including manual therapy, therapeutic exercise, and modalities.
Ability to work independently and as part of a healthcare team.
